A health summary is a comprehensive overview of a patient's health information and medical history. It helps healthcare providers have a clear understanding of a patient's health status, supporting safe and coordinated care.  

What information is in a health summary?

A typical health summary may include:  

Personal details: Basic information such as name, date of birth and contact details.

Medical history: A record of past medical conditions, surgeries and hospitalisations.

Medications: A list of current and past medications, including dosages and duration.

Allergies and adverse reactions: Known allergies or adverse reactions to medications.

Immunisation history: Details of immunisations received, including dates and types of vaccines.

Family history: Relevant health conditions present in immediate family members.

Social history: Lifestyle factors such as smoking, alcohol consumption and exercise habits.

Current health status: Details of current health conditions, symptoms and ongoing treatments.

How to get a copy of your health summary

You can request a copy of your health summary from your regular GP. When requesting your summary, it’s helpful to ensure it includes:

  • Current and past medical conditions
  • Complete medication and allergy lists
  • Recent diagnoses (within the last three months)

If your health summary lacks this essential information, we may request that you revisit your GP to have it updated.  

Submitting your health summary

You can upload your referral and health summary when completing your intake form. If you don’t have your documents ready, our clinical team will let you know what to provide and how to access them, so the process keeps moving smoothly.
