- Get started with our digital intake form
Your journey begins with Candor’s digital patient intake form, designed to help us understand your healthcare needs.
- Begin by selecting the service you require and complete the intake form.
- This form helps our clinical team understand your medical history and ensure we have the information needed to prepare for a smooth and informed telehealth consultation.
*Please note, it does not guarantee a prescription.
- Intake review and documentation
Once submitted, our clinical team carefully reviews your intake form to confirm we have the right information to support a safe and effective consultation.
- If needed we may request additional information, such as a health summary or referral or letter of support from your regular GP.
- For identity verification, a valid driver’s license or government-issued ID is required to be presented.
- Once everything is complete you will be sent a link to book your first consultation with an Australian-registered clinician.
- Your telehealth consultation
Your Candor telehealth consultation allows you to meet one of our registered Australian clinicians from the comfort of your home.
- During your consult, your clinician will review your medical history, current health goals and any relevant documentation you provided during the intake review.
- You will discuss possible treatment options and whether a prescription is appropriate.
- You are welcome to share any previous treatment experiences or preferences, but prescriptions are issued solely at the clinician’s discretion.
- Prescriptions
If a prescription is deemed appropriate, it will be issued and managed securely through your Candor account.
- All medicines are dispensed by independent, licensed Australian pharmacies. Candor does not operate a pharmacy.
- For your convenience, delivery is available through our partner pharmacies. Shipping fees and timeframes may vary depending on your order and location.
- You can track your prescription status and manage your medication directly from your Candor account.
- Ongoing care
Candor supports your health journey beyond your initial consultation.
- Your Candor account provides a central place to manage your consultations, follow-up appointments, prescriptions and chat access with our clinical team.
- If your circumstances changes or you have questions, you can contact our team through your account for support.
